충북도의 대표 의료복지 정책인 의료비후불제 지원 대상이 한부모가족까지 확대된다. 융자 한도도 최대 500만원으로 상향된다.
김영환 충북지사는 11일 충북도청 브리핑룸에서 기자회견을 열고 다음달부터 의료비후불제 지원을 확대한다고 밝혔다.
◆ 지원 대상 확대
종전까지 65세 이상 노인, 기초생활수급자, 차상위계층, 장애인, 국가유공자, 2자녀 이상 다자녀가구가 적용 대상이었다. 다음달부터 한부모가족도 혜택을 받을 수 있다.
이번 조치로 도민 2만명이 새롭게 의료비후불제를 이용할 수 있을 것으로 추산된다. 기존 적용자까지 합하면 수혜 대상자는 도 인구의 절반인 83만명까지 늘어난다.
충북도에 따르면 도내 한부모가족은 2만여명 정도다. 충북 지역 인구는 159만2431명이다.
◆ 융자 한도 상향과 지원 범위 확대

융자 지원 한도는 기존 최대 300만원에서 500만원으로 늘어난다.
14개 대상 질환에 대한 포괄적 지원도 이뤄진다. 그동안 대상 질환의 시술·수술비만 융자 지원을 받을 수 있었지만, 앞으로 진료비까지 지원 범위가 확대된다.
김영환 지사는 "모든 질환에 대해 165만 전 도민이 이용할 수 있도록 제도를 확대할 계획"이라고 밝혔다. 김영환 지사는 "우선 신용불량자까지 대상을 늘리는 방안을 정부와 협의 중"이라고 설명했다.
◆ 제도 운영 현황과 전국 확산
의료비후불제는 충북도가 2023년 1월 전국 최초로 도입한 선순환적 의료복지제도다. 목돈 지출 부담으로 제때 치료를 받지 못하는 취약계층을 위해 농협이 먼저 대납한 의료비를 이후 3년 동안 분할 상환하는 방식이다.
대출이자는 충북도가 부담하고 원금 회수가 안 되면 충북도가 대신 갚은 뒤 직접 회수한다. 금융권 연체 중이거나 신용불량자는 이용할 수 없고 연간 이용 횟수는 한 번이다.
제도를 시행한 2023년 1월 이후 10일 현재까지 2210명(57억293만원)이 이용했다. 상환률은 99%로 높아 성공적으로 안착하고 있다.
시행 초기 80여곳이던 참여 의료기관도 충북대병원, 청주성모병원 등 도내 종합병원급 13곳, 병원 21곳, 개인의원 263곳 등 297곳으로 늘었다.
전국 지방자치단체의 관심도 높다. 서울시는 내년부터 서울형 의료비후불제를 도입한다. 서울시는 지난해 12월 충북도와 의료비후불제 도입을 위한 협약을 체결했다. 서울시는 내년 상반기에 의료비후불제 시범사업을 시행하기 위해 조례 개정 등 행정 절차에 착수했다.
경기도의회는 의료비후불제를 도입하기 위해 조례 제정을 추진하고 있다. 전국 지방자치단체의 벤치마킹도 이어지고 있다.
김영환 지사는 "사회적 약자의 실질적인 건강 보호를 위한 의료비후불제가 궤도에 오르고 있다"고 밝혔다. 김영환 지사는 "제도의 효과성과 지속 가능성을 면밀히 살펴 충북을 넘어 전국으로 확대될 수 있길 기대한다"고 말했다.
김영환 지사는 "의료비후불제가 대한민국 전체로 확대되길 희망한다"며 "의료비후불제를 의료복지의 새로운 표준으로 세워가겠다"고 말했다.

◆ Expansion of Support Recipients

◆ Loan Limit Increase and Support Scope Expansion
The loan support limit will increase from the existing maximum of 3 million won to 5 million won.
Comprehensive support for 14 target diseases will also be provided. Previously, only procedure and surgery costs for target diseases could receive loan support, but going forward, the support scope will expand to include medical treatment costs.

◆ System Operation Status and Nationwide Expansion
The Medical Deferred Payment System is a virtuous cycle medical welfare system that Chungbuk Province introduced nationwide for the first time in January 2023. It is a method where Nonghyup first pays medical expenses on behalf of vulnerable groups who cannot receive timely treatment due to the burden of lump-sum expenditures, and patients repay them in installments over the next three years.
Chungbuk Province bears the loan interest, and if principal recovery is not possible, Chungbuk Province repays it instead and then directly recovers it. Those in arrears with financial institutions or credit defaulters cannot use it, and the annual number of uses is once.
Since the system was implemented in January 2023, 2,210 people (5.7029 billion won) have used it as of the 10th. The repayment rate is high at 99%, indicating successful establishment.
Participating medical institutions have increased from about 80 at the beginning of implementation to 297, including 13 general hospitals in the province such as Chungbuk National University Hospital and Cheongju St. Mary's Hospital, 21 hospitals, and 263 private clinics.

The Gyeonggi Provincial Council is promoting ordinance enactment to introduce the Medical Deferred Payment System. Benchmarking by local governments nationwide continues.
